Without question the most-repeated theme of all; I count 38 appearances across the three movies. It is the signature theme for the first movie (that is, it sounds over the title), as Rohan is for the second and Gondor for the third. Where the Shire theme is placid and soothing, Fellowship is heroic, jagged and assertive. The three strong chords that open it (I-flat III-I) are often used by Shore to suggest the whole.

Occurrences:

Over the title: "The Fellowship of the Ring".  

The scarecrow scene. Sam takes the step that separates him from the Shire; Bilbo's voiceover: "It's a dangerous business going out your door." As Frodo and Sam leave the Shire, the first elements of the fellowship have formed, and Shore gives us a suggestion of the theme; the full statement will wait until the Council of Elrond, when all nine members are gathered for the first time.

Gandalf gallops to Isengard. Another hint, this time in a modal form in the horns.
Hobbits trudging cross-country with Aragorn; the fellowship is now up to five. Shore gives us a bit more of the theme.
The Council of Elrond. Gandalf: "I will help you share this burden"; the others join in.  
Elrond names them The Fellowship of the Ring. The first full statement of the theme.
The fellowship sets out from Rivendell. A pensive, reharmonized version, then the full theme.
Moving single file past the rock on the ridgetop [full statement].  
The bridge of Khazad-Dum. The theme is heard several times during this sequence.

The death of Boromir. Aragorn: "Be at peace, son of Gondor." Here Shore gives us a wonderful "broken" version of the theme, altering the harmonies to cadence early, signifying the breaking of the fellowship.

First shots of Aragorn, Gimli and Legolas in pursuit of the Uruks. The theme sounds several times during this sequence.  
Rejoining the chase of the Uruks [combined with Isengard].
